Thank you for taking the time to help us better understand how to serve you, your family, and your schooling community. This survey should take approximately five minutes to complete. All of your responses will remain completely anonymous. At the end of the survey, you will be asked to complete a section containing personal information. This information is only for the purpose of managing your participation in the study so you may be entered into the raffle for free tutoring, or be contacted to participate in a focus group, if you would like to further contribute to this study (you will be able to opt-in to the focus group at the end of the survey). However, the participant list will be kept separately from your answers. This will assure that your name is not attached in any way to your responses. Any personal information that is gathered will be used only in consideration of the free tutoring drawing and will then be destroyed.The winner of the free tutoring will be notified via email in the next 4-6 weeks.
